청소년 성범죄자의 정신병리에 따른 유형 분류와 치료적 효과의 차이
본 연구의 목적은 청소년 성범죄자의 유형을 정신병리적 특성에 따라 분류하고, 인지행 동치료프로그램을 적용한 결과가 각 집단 유형에 따라 어떤 차이를 보이는지를 탐색하 는데 있다. 2003년부터 2010년까지 성폭력 가해 행동으로 보호관찰 및 소년원 입원 중 인 남자 청소년 288명을 대상으로 한 자기보고식 설문 결과를 분석하였다. 청소년 자 기행동평가척도의 내재화 문제행동 척도와 외현화 문제행동 척도에서 T점수 60점 이상 의 획득을 기준으로 하여, 하위 집단은 정상/방어 집단 198명, 내재화 집단 16명, 외현 화 집단 40명, 혼재 집단 34명으로 구분되었다. 성폭력 가해 청소년에 대한 집단인지행 동치료 프로그램을 적용한 결과, 내재화 문제 중 우울감과 특질불안에서 치료 효과가 유의하였고 특히 외현화 집단과 혼재 집단에서 더 긍정적 효과가 있었으며, 외현화 문 제 중 특성분노와 상태분노 등 분노감의 경험에는 치료 효과가 유의하였으나 분노감 표현에 있어서는 치료 효과가 유의하지 않았다. 성폭력에 대한 인지적 왜곡에 있어서 는 치료 효과가 유의하였으며, 치료 효과에 있어서의 집단 간 차이는 유의하지 않았다. 추가적으로, 정신병리적 특성에 따른 유형분류가 치료적 개입에 미칠 수 있는 긍정적 효과와 향후 인지행동치료프로그램에 적용되어야 할 개선점에 대해 논의하였다.
Classifying juvenile sex offender subtypes and the treatment outcome in accordance with their psychopathology
The aim of this study is to classify subtypes of juvenile sex offenders (JSOs) in accordance with their psychopathology and to compare the treatment outcome of cognitive behavioral treatment (CBT). The results of self-report questionnaire of 288 male juvenile sex offenders who were in probation center or juvenile training school in Korea between 2003 and 2010 were analyzed. The subtypes of JSOs were classified with the T-score of 60 of internalizing and externalizing behavior problem subscales of Korea Youth Rating Scale: normal/defensive subtype (n=198), internalizing subtype (n=16), externalizing subtype (n=40), and mixed subtype (n=34). In terms of the treatment outcome of group CBT, depression and trait anxiety decreased, especially in externalizing and mixed subtypes. Anger experience were improved, where as the expression of anger were not, and cognitive distortions about sexual offending were modified after CBT. The importance of these findings for the role of therapist of CBT and enhancing JSOs’ motivation to participate in treatment is discussed.
